The cost is $61/adult resident plus an additional $10 for each other state you have lived in.  All adults residing in the unit must submit an application. Application fees are non-refundable once your application has been processed. No applications will be processed until payment is received.

Please make sure your applications are filled out completely and signed.  Failure to do so will delay processing.

Be honest on your application and feel free to attach explanation or documentation. False or omitted information will lead to a denial of your application.
